SMP Health - St. Raphael, a non-profit, faith-based nursing home hosts 170 beds, and has served the Barnes County and Valley City area for over 85 years. SMP Health – St. Raphael is sponsored by the Sisters of Mary of the Presentation, a Catholic health care system that offers a diversity of programs and facilities across North Dakota and Illinois. We serve those in our care with respect and compassion as we strive to fulfill the healing mission of Jesus.

Our services include short and long-term care options for individuals needing assistance with skilled nursing, rehabilitation, mental illness, Alzheimer’s and dementia care, and outpatient Lifeline support.

SMP Health – St. Raphael is a Medicare/Medicaid/VA contract participant.

Skilled Nursing
Our staff provides compassionate care through 24-hour skilled services.
Alzheimer’s Unit
The Circle of Life Cottage is a 18 bed unit created to provide quality care in a home-like environment to those with dementia or Alzheimers.
Rehab & TCU
We provide Physical, Occupational, and Speech therapy in our state of the art fitness Wellness Center to prepare an individual for a successful return home or to another care setting.
Sycamore Special Care Unit
Our compassionate healthcare services focus on meeting the needs of all individuals’ psycho-social needs in a safe and secured home-like environment.
